Chong Hua Hospital now offers the PhilHealth Z Benefit for Breast Cancer for qualified members and dependents. The package provides coverage for approved services based on your condition, treatment plan, and PhilHealth evaluation.

Who Is Eligible

Qualified PhilHealth members and dependents diagnosed with breast cancer are eligible for evaluation.

A diagnosis alone does not guarantee approval. Your attending physician will assess your condition and recommend a treatment plan. The hospital submits the required documents to PhilHealth Region 7 for approval before covered Z Benefit services are provided.

Services and Package Amounts

Covered services depend on your breast cancer stage and approved treatment plan. Your attending physician will explain which services apply to your condition.

Diagnostic and Prognostication Services

These tests help doctors assess the cancer and plan treatment. Package amounts range from ₱1,000 to ₱10,000.

Breast Cancer Surgery

Package amounts range from ₱30,000 to ₱140,000, depending on the approved procedure.

Package amounts range from ₱122,000 to ₱198,000 for a complete cycle.

Hormonal Therapy

Package amounts range from ₱5,400 to ₱36,000 for a complete cycle.

Targeted Therapy

The listed package amount is ₱1,000,008 for a complete cycle.

Surveillance Services

These services involve follow-up monitoring. Package amounts range from ₱300 to ₱4,000.

These figures are package amounts, not cash payments to patients. Not every patient needs or receives every service listed.

The patient information guide uses “complete cycle” for the treatment amounts above. Ask your care team to explain the treatment schedule and services included in your approved package.

How to Apply

Step 1. Consult Your Attending Physician

Visit your Chong Hua Hospital accredited Primary Attending Physician and obtain an endorsement for Z Benefit evaluation.

Step 2. Submit Your Test Results

Submit the complete diagnostic and prognostication test results to the Hema-Onco Unit Nurse Coordinator. Your care team will guide you on the requirements.

The patient guide lists mammogram or ultrasound, biopsy, complete blood count with platelet count, ER/PR hormone receptor testing, alkaline phosphatase, HER2/neu testing through immunohistochemistry with FISH confirmation if needed, and a metabolic panel with liver function tests.

Step 3. Wait for Your Case Review

The Multidisciplinary Team, or MDT, will review your case and treatment plan. The Hema-Onco Unit Nurse Coordinator will notify you of the result.

Step 4. Submit Your Treatment Documents

Submit the MDT Plan and Endorsement Slip to the Z Benefit Coordinator at the Medical Records Office.

Step 5. Complete the Pre-authorization Documents

Complete the required PhilHealth Z Benefit pre-authorization documents at the Medical Records Office.

Step 6. Wait for PhilHealth Approval

The Z Benefit Coordinator will notify you once PhilHealth Region 7 approves your pre-authorization.

Step 7. Proceed with Admission or Treatment

Once approved, proceed with admission or treatment according to the arrangements with your care team.

Approval and Payment Reminders

Pre-authorization means approval from PhilHealth before covered Z Benefit services are provided. Approved pre-authorization is valid for 60 calendar days from the approval date.

A co-payment is the amount you pay toward your covered care. According to the patient information guide, the co-payment will not exceed the package rate. Your Primary Attending Physician will discuss the arrangement with you.

If a co-payment applies, the required amount must be fully settled upon admission or treatment. Ask your physician and Z Benefit Coordinator to explain your approved coverage and payment responsibilities.
