The Clinical Nutrition Service of Chong Hua Hospital offers a wide range of inpatient and outpatient nutrition consultations and education sessions for people interested in learning how to eat healthy, understand nutrition labels, and how to incorporate all foods into a healthy eating plan for various medical conditions such as:

Our hour-long, two-part, one-on-one consultation is intended to achieve the following goals:

1. To integrate positive lifestyle change and eating habits
2. To motivate patients to include physical activity as part of their lifestyle
3. To support the short and long-term maintenance of the program by providing effective dietary recommendations and meal plans
4. To monitor and evaluate adherence on the set timeline through follow-up visits

1. Nutrition Assessment and Goal Setting
   a. Anthropometric Screening every consultation (Weight, BMI, and Waist Circumference)
   b. Clinical Assessment (Nutrition – Focused Physical Assessment)
   c. Laboratory Assessment ( Nutrition – Related)
   d. Dietary Assessment ( Food Recall, Calorie and Nutrient Counting)

2. Comprehensive Nutrition Counseling
   a. Evidence – based Dietary Management
   b. 1 Day Meal Plan & Sample Menu
   c. Nutrition Guidelines ( Nutrition Education Brochures)

3. Physical Activities/Exercise Recommendations
   a. Planned and structured physical activity specific to types or classifications of diabetes and/or other disease Condition

4. Monitoring and Evaluation
   a. Advise follow-up visit to evaluate progress and/or implement modification
   b. Provision of free follow-up visit for minor modifications

At Chong Hua Hospital Cebu, comprehensive dietary programs and instructions for Diabetes, Thyroid and Metabolic Disorders are available at the Multidisciplinary Center at 2nd Floor Main Building .

At Chong Hua Hospital Mandaue, all dietary instructions are provided on the 10th floor of the Medical Arts Building in the Wellness Center.

Requirements:

• Prior to visit, secure Diet Prescription/Referral from Physicians
• Patient are advised to bring their laboratory results
• For patients without a Physician’s Diet Prescription or Referral, they will be evaluated, and nutritionally managed based on the dietitian’s assessment.

This includes the following consultation on:

• General Health and Wellness
• Weight Management
  · Patients with existing clinical conditions/co-morbidities will be referred back to their Physicians for referrals.
