1. What information is required to reserve a hospital room?

Patient and/or their significant other shall provide the following details:

  • Date of admission
  • Patient’s full name (with middle name)
  • Patient’s contact details
  • Date of birth
  • Reason for admission
  • Attending physician
  • Room preference
  • Branch preference
  • Contact person
  • Contact number
  • HMO/insurance (if applicable)

2. How is room availability determined during admission?

Room and bed assignments are based on the date of admission. The admitting staff records these requests and allocates rooms accordingly, considering both patient preferences and medical requirements.

3. Is a partial payment required upon hospital admission?

  • Chong Hua Hospital requires an initial partial payment for all admissions. In the case of a scheduled, non-urgent admission, the payment must be made upon arrival. For urgent or emergency cases, the partial payment must be settled once the patient’s condition has stabilized. Our admitting staff will provide you with the necessary information.
  • If you have a valid Letter of Authorization (LOA) or Letter of Guarantee (LOG) from your insurance provider, HMO, or affiliated company, you will not be required to make a partial payment. If an LOA/LOG is not available at the time of admission, a partial payment is required, which may be refunded upon discharge.
  • During your hospital stay, you will be asked to make progressive payments corresponding to the increasing bill amount. An updated statement of your account can be viewed via the online patient portal.

4. Can Chong Hua Hospital assist with hospital transfers?

  • If you require an inter-hospital transfer, Chong Hua Hospital will assist in facilitating the transfer.
  • If you decide not to proceed with inpatient admission, Chong Hua Hospital will respect your decision and assist in transferring you to your preferred hospital.

Inter-Hospital Transfer

  • The patient or their significant other will be advised to coordinate with both the receiving hospital and Chong Hua Hospital.
  • The patient or their significant other will be advised to let the endorsing doctor and nurse call the ER Station for the initial endorsement of the patient and to check for bed availability. If the patient is admitted in one of the healthcare facilities in Region VII, the endorsing hospital will be asked to encode and make the DOH online referral.
  • The endorsing hospital shall coordinate with the receiving hospital through doctor-to-doctor endorsement.
  • The patient or their significant other will also be asked to call the admitting section to be appraised on the financial side of the admission.
  • Room category will depend on the present condition of the patient.

5. Will I be informed of my assigned hospital room number before admission?

No, your assigned room number will be disclosed only to you and your significant other upon admission.

8. What are the requirements for Direct-to-Room (DTR) admission?

For Direct to Room admission, please be reminded of the following:

  • Direct to Room orders are valid for 24 hours. Orders exceeding this timeframe require reassessment by the resident doctor.
  • Have the direct-to-room admission order signed by your Attending Physician.
  • Direct-to-room order should have the following details:
    • Patient’s full name with middle name
    • Diagnosis
    • Procedure or planned surgery if applicable
    • Date of Admission
    • Date of Issuance
    • “Direct-To-Room” phrase
    • Attending physician’s signature

9. What are the visitor and watcher guidelines at Chong Hua Hospital?

  • Non-COVID patients are permitted a maximum of two healthy adult visitors or watchers during admission.
  • For a confirmed COVID patient, one adult watcher is permitted during admission.
  • For non-COVID patients, except those in the Critical Care Units, you may have healthy visitors from 6 AM to 9 PM.
  • All visitors must observe physical distancing.
  • All visitors must wear a surgical mask at designated areas within the hospital (critical care units, emergency room, isolation rooms).
  • All visitors must practice cough etiquette. Ensure that a surgical mask is on when you cough or sneeze. Change the mask if it is soiled or damp. Throw the used mask in the yellow trash bin and wash your hands afterward.
  • Visitors must follow proper hand hygiene protocols such as frequent hand washing and the use of hand sanitizers.
  • Visitors experiencing symptoms of COVID-19, such as fever, cough, and difficulty breathing, shall not be allowed to visit patients.

Watcher/Visitor

  • Chong Hua Hospital requires patient watchers to be able-bodied adults. For the safety of both patients and visitors, minors and seniors are discouraged from serving as watchers. This policy is in place due to the increased risk of illness and falls for children and seniors within the hospital environment. However, minors (7 years old and above) and seniors are welcome to visit during designated visiting hours, provided they adhere to infection control guidelines.

2. How do I schedule a consultation and procedure at the Chong Hua Hospital Aesthetics Center?

  • The patient must be first consulted by the doctor affiliated with CHH Aesthetics Center. Then, the doctor will schedule the desired procedure.
  • We highly recommend that the patient contact us through text at least a day before the visit so that the unit can send the patient the detailed schedule of the assigned doctor.
  • Consultation is on a first-come, first-serve, walk-in basis only. No appointments.

3. What is the HMO coverage as of January 2025 per doctor?

  • CHHM Aesthetics Center accepts insurance. However, there will be a facility fee of P225.00 per consult.
  • Procedures are subject to HMO coverage and approval. The patient must call the Aesthetics Center first to check coverage for insurance.

4. Can I schedule an aesthetic procedure immediately after my consultation?

  • Please be advised that the Aesthetics Center may not be able to schedule a treatment immediately following your consultation, as our schedule is usually booked in advance by other patients.
  • We highly recommend the patients come as early as possible since we might have multiple procedures on that day. Scheduled procedures for the day are still on a first-come first-served basis.

5. Other FAQs:

  • Patients must consult with affiliated Aesthetics Center Dermatologists before any procedures
  • Aesthetics Center offers a limited line of products for sale.
  • Nurses and other staff cannot recommend treatment to patients.
  • All procedures except for basic facials are performed by Board Certified Specialists Dermatologists
  • For treatment sessions, the number of sessions will vary depending on your skin’s condition and its response to the treatment. Please talk to your doctor.

Yes, Chong Hua Hospital accepts both international insurance and local HMOs, subject to accreditation and coverage policies:

  • International Insurance – If your international insurance is accredited with Chong Hua, the covered amount will be directly deducted from your bill. If not, we can advise for reimbursement, or help find a local handler that is accredited with us.
  • Local HMO – You can use multiple HMOs (more than 2) with a Letter of Authorization from each. Coverage depends on the HMO’s calculation. We can check if your HMO is on our list.

4. Does Chong Hua Hospital accept government assistance and discounts?

Yes, Chong Hua Hospital accepts various forms of government assistance and discounts:

  • We accept LOA from DSWD and PCSO.
  • We also accept DOH MAIP and CHAMP and other government agencies on a case-to-case basis.
  • PhilHealth coverage depends on the final diagnosis of the patient. It can be availed as Outpatient with minor operation/procedure. Admission should be at least 24 hours in order to avail of PhilHealth discount.

1. What is Cardiac Rehabilitation?

Cardiac Rehabilitation is a medically supervised program designed to improve your cardiovascular health if you have experienced a heart attack, heart failure, angioplasty, or heart surgery. It includes exercises and health education.

3. Which Cardiac Rehabilitation program will I be enrolled in?

  • Phase 1: 8 sessions + treadmill stress test after 8th session
  • Phase 2: 24 sessions + treadmill stress test after 24th session
  • Phase 3: 48 sessions + treadmill stress test after 48th session

Staff will send an SMS/text message when your results are ready for pickup.

7. How long does the program take?

Typically 1 to 3 months, with each session lasting ~1 hour. Sessions are recommended three times a week with one-day intervals.

8. How should I prepare for a session?

  • Eat a light meal; bring water

2. Is a written prescription required for cardiovascular procedures?

Yes. All procedures must have a written prescription from the attending/referring/requesting physician.

3. How do I schedule a cardiovascular procedure?

  • Scheduling must be done by the performing doctor (Cardiologist for Cath Lab, CV Surgeon for CVOR).
  • Both inpatients and outpatients are accepted.
  • Patients must be evaluated by their doctor to determine eligibility for outpatient procedures.

4. How should I prepare for my procedure?

  • Fasting: NPO after midnight for AM procedures; light breakfast for PM procedures then NPO 4–6 hours.
  • Arrival: Be at the unit at least 1 hour before schedule (for OPD).
  • Companion: At least 1 S.O. must accompany the patient.
  • Bring Lab Results:
    • CBC
    • PT
    • ECG
    • 2D Echo
    • Creatinine
    • RAT (if symptomatic)

6. How can I request a price quotation?

1. What is a sleep study, and where is it done at Chong Hua Hospital?

A sleep study or polysomnogram (PSG) is a test used to diagnose sleep disorders. It is conducted overnight in a private sleep lab room.

  • Cebu: 11th Floor, Building AB

2. What types of sleep studies are offered?

  • Diagnostic Overnight PSG: Monitors EEG, ECG, eye movements, oxygen, breathing, and limb movement.
  • Split-night PSG with CPAP Titration: First half diagnostic; second half CPAP adjustment if sleep apnea is confirmed.
  • CPAP Titration Study (Therapeutic): Determines best pressure setting for CPAP therapy.

3. How do I schedule a sleep study?

  • Doctor’s prescription is required before booking.
  • If no physician, the Pulmonary Unit can assist with referrals.
  • To cancel or reschedule, notify the unit at least 3 days in advance.

4. How long does it take to get results?

Results are available within 1–2 weeks. Data is scored by a sleep technician and interpreted by a sleep doctor.

5. How can I access my results?

Results may be claimed at the Pulmonary Unit or emailed to provincial/out-of-town patients by the Unit Manager.

6. How long does a sleep study take?

Typically 8:00 PM to 6:00 AM. Sleep is encouraged early for maximum observation time.

7. How should I prepare for my sleep study?

  • Shower before arriving; no lotions, creams, or hair products
  • Wear/bring comfortable sleepwear
  • No naps or physical exertion before test
  • Avoid caffeine, alcohol, and sleeping pills
  • Have dinner beforehand
  • Bring your doctor’s prescription and approved charge slip
  • Only pediatric patients may have a companion stay in-room
  • For insured patients, secure a valid LOA before the procedure

8. What happens during a sleep study?

Technicians will place non-invasive sensors and monitors on your scalp, face, legs, chest, and finger to measure brain waves, breathing, oxygen, and muscle activity throughout the night.

9. Is the sleep study covered by HMO or PhilHealth?

Many insurance plans partially cover sleep studies. Pre-approval or LOA may be required. Contact your provider to verify eligibility and benefits.

2. How do I schedule an obstetric procedure?

Walk-in patients are accepted for both OPD and inpatient procedures with a physician’s prescription.
Elective Cesarean Sections are scheduled by attending physicians, OB residents, or clinic secretaries.

3. How long do procedures take?

8. What is fetal monitoring?

Fetal monitoring checks the baby’s heartbeat and vitals using CTG or fetal Doppler during pregnancy/labor.

9. Where should I go for NST?

Proceed to the ER; staff will coordinate with the Delivery Room team.

10. COVID-19 Test for Pregnant Patients

Rapid Antigen Test is required within 24 hours if symptomatic (fever, cough, colds).

11. Will my birth plan be honored?

Yes. Plans are flexible and can be revised during labor if needed.

13. Can my husband attend my delivery?

Yes, for low-risk vaginal births. Requires consent and submission of the Request to Attend Delivery Form two weeks before EDD.

14. Can I take photos/videos?

Photos are allowed after cleaning the baby. Videos are strictly prohibited. Staff consent is needed if they appear in photos.

15. Is a swab test required for SO/guests?

Symptomatic individuals (fever, cough, colds) are not allowed in the DR complex.

16. How long is the recovery time post-delivery?

Usually 2 hours in recovery before transfer to room, depending on condition.

17. What if I have cord blood?

Inform your OB and DR staff in advance to arrange collection.

18. When should I go to the labor room?

If you experience bloody discharge, contractions every 3–5 mins, or water breaks. Go to ER then DR Complex.

19. What is CTG/NST?

Cardiotocography monitors fetal heartbeat and contractions. Duration: 30 mins – 1 hour.

20. Why is CTG done?

To assess fetal well-being and identify early signs of distress before complications arise.

21. Can someone assist me with admission if no SO is available?

Yes. Patient Assistant Specialists (PAS) can help with your admission process.

22. How long will I stay in recovery?

About 2 hours, depending on doctor’s assessment.

23. What should I bring?

  • Mother’s Book or OBS Card
  • Latest Ultrasound and Lab Results

24. Can I bring my phone in the DR?

No. Phones are prohibited in labor and delivery rooms per Data Privacy Act of 2012.

25. When will my baby be roomed in?

After 4 hours if healthy. If not, baby will be admitted to NICU.

26. Can I return unused items?

Yes, before discharge or settling your account.

27. Can I bring religious items?

Yes, if not bulky or valuable.

28. Can I use the comfort room while in labor?

Only if not in active labor or ruptured. Otherwise, a bedpan will be provided.

  • Yes. Consultation is required regardless of referral. Scheduling is coordinated by the referring facility or physician.

For Patients with a Referral:

  • A gastroenterologist consultation is required before proceeding with the procedure, even with a referral.

For Patients Referred from Another Hospital:

  • If the procedure is not available at the referring hospital, a gastroenterologist consultation is still necessary before scheduling.
  • After the family selects a gastroenterologist and completes the consultation, the resident physician must coordinate with the gastroenterologist for further instructions.

8. Important Reminders

  • Chong Hua Hospital (CHH) HMO cannot issue an LOA to all insurances, in which Outpatient benefits are clinic-based.
  • Requesting doctors must be accredited with insurance.
  • Society of NEUROLOGY and UROLOGY are not affiliated with any insurances, with this, the Issuance of LOA for Consultation is not possible.
  • Before obtaining the Letter of Authorization (LOA), please ensure that you know your doctor, as their information is needed for the issuance of the LOA.

7. What are the preparations before the procedure?

  • Wear soft-soled shoes and comfortable clothes
  • Bring extra shirt, face towel, and bottled water
  • Avoid caffeinated drinks (coffee, soda, tea, energy drinks)
  • Do not take beta blockers a day prior (e.g. Verapamil, Atenolol, etc.)
  • Post-bypass/angioplasty/ACS patients may continue beta blocker maintenance

We provide a wide range of treatments and supportive care for cancer patients at both of our locations.

Chemotherapy (Available at Both Sites)

What are the different types of chemotherapy to target cancer:

  • Intravenous Chemotherapy (Through the vein)
  • Intrathecal Chemotherapy (Into the spinal fluid)
  • Intramuscular Chemotherapy (Into the muscle)
  • Intravesical Chemotherapy (Into the bladder)
  • Subcutaneous Chemotherapy (Under the skin)

Other Cancer Treatments

  • Immunotherapy – Boosts the body’s natural defenses against cancer
  • Chelation Therapy – Helps remove heavy metals from the body
  • Therapeutic Phlebotomy – A procedure to remove extra blood when needed
  • Pulse Therapy – High-dose treatment given in short bursts

Blood Transfusion (For Cancer Patients Only)

Medications and Fluid Support

  • Chemotherapy Tablets – Oral chemo medication
  • Hydration Therapy – Extra fluids before a CT scan, MRI, or PET scan with contrast
  • IV Antibiotics (For Cancer Patients Only) – Given through the vein when needed

2. Is a written prescription required?

To ensure patient safety and proper preparation, a written prescription from your Attending or Requesting Physician is required before any procedure. The physician must be affiliated with our unit.

  • For Tumors – Oncologists
  • For Blood Disorders – Hematologists
  • For Rheumatic Disorders – Rheumatologists
  • For OB-GYN Cases – OB-GYN Oncologists only
  • For Surgical Cases – Surgical Oncologists only

This policy helps ensure that each procedure is performed correctly and safely, tailored to the patient’s specific condition.

3.How do I schedule or reschedule a treatment or procedure?

  • Your Attending Physician will determine and schedule your treatment or procedure.
  • If you need to reschedule, please coordinate directly with your Attending Physician.
  • Walk-ins are highly discouraged to ensure proper preparation and patient safety.

4. How long does it take to get the results of a Bone Marrow Aspiration and Biopsy?

  • If done within Chong Hua Hospital: Results will be available in 7 working days.
  • If sent to an external facility (NKTI or SLMC): Results will be available in 12-14 working days.
  • For result availability, please contact the Laboratory Releasing Unit.

5. Where to Access the Results (Bone Marrow Aspiration and Biopsy Results)?

Where to Claim:

  • For tests done within Chong Hua Hospital: Histopathology Department
  • For send-out tests (NKTI or SLMC): Laboratory Releasing Area

Important Reminders:

  • Bring the patient ledger/receipt when claiming results.
  • If a representative is claiming the results, a signed Authorization Letter is required.

  • Can be done by a Doctor (OBGYNE) in their clinic, or at the OPD Unit.
  • If patient opts to proceed to OPD Unit, follow these steps:

(1) Proceed to the laboratory for charging. A charge slip will be given to you. Once you have the charge slip, proceed to billing.

(2) At the Billing dept., present your charge slip and pay for the procedure. Once you have the official receipt, proceed to the Histopathology unit.

(3) At the Histopathology unit, present your Official Receipt in order to claim your pap smear kit. Once you have the kit, proceed to OPD.

(4) At the OPD unit, inform the unit in charge of your procedure. If there is no available slot for the day, a staff member will inform you of the next available schedule.

(5) After the procedure, bring your kit (specimen) back to the Histopath Unit. The result is available 7 working days after the specimen is submitted. Note: Result is not available to Patient Portal, the result is for pick up.

3. Do I need a written prescription?

Some laboratory tests require a written prescription and prior assessment from the attending physician, while there are also tests that can be availed as per patient’s preference.

4. How to avail your services?

  • Walk in. First come, first serve basis
  • DRIVE THRU patients may walk in.

5. What is the standard turnaround time for laboratory results?

  • Papsmear – Papsmear kit will be given to the patient. The patient may opt to have the procedure performed either at the OPD or at her OB Gyne’s Clinic. The patient shall personally submit the kit with the specimen at the lab. The result is available 7 working days after the specimen is submitted.
  • Bone Marrow biopsy – Results are released within 5-7 working days.
  • 2nd opinion tests – Cases wherein patients submit their samples/specimens for 2nd opinion. (e.g. procedures performed in other hospitals). Results are available within 5-7 working days.
  • Peripheral Blood Smear Test – Results are available within 5-7 working days.
  • Frozen Section Biopsy – STAT procedure. Result is available after 30 mins.

6. How can I access my results?

  • You may view your result on our patient portal https://chi.chonghua.com.ph/. All lab test results are available in the portal except results for biopsy, HIV and special exams.
  • For pick-up of results, you need to present the patient’s valid ID and official receipt.

For authorized representative, you need to present the ff: upon claiming of results:

  • Letter of Authorization from the patient
  • Patient’s Valid ID
  • Official Receipt of the Laboratory Test
  • Valid ID of authorized person to claim
  • If the patient is unable to pick up the result or if the result is not available in the patient portal, we can email the result provided that you will completely fill up the email consent form. We will send the result via email, provided that the Email Consent Form is duly completed and submitted to chhc_laboratory_@chonghua.com.ph.

7. What are the preparations needed?

  • Glucose Fasting/Fasting Blood Sugar (FBS): 6-8 hrs

  • The Certificate of Live Birth will be processed at the Medical Records Office.
  • For babies born to married parents, the Medical Records staff will assist the parents in registering the Certificate of Live Birth with the City Civil Registrar’s office. The parents can collect the registered Birth Certificate 2 to 3 weeks after the baby’s discharge.
  • For babies born to unmarried parents, the parents must personally register their baby’s Certificate of Live Birth at the City Civil Registrar’s office. Parents are required to submit a notarized affidavit as well as a notarized Certificate of Live Birth to the City Civil Registrar’s office. Affidavit will be provided by the Medical Records staff.

5. What are the prices and preparation guidelines for Nuclear Medicine procedures?

  • All Female patients that are breastfeeding, childbearing, who suspect pregnancy and or possibly pregnant should notify the Nuclear Medicine Staff for further assessment.

Below are Nuclear Medicine procedures with their prices and preparation instructions:

  • Bone Scan
    • 4 weeks before the procedure – Thyroid hormone (e.g., Eltroxin, Thyrax, Euthyrox)
    • Price: P8,605.00
    • No fasting required prior to bone scan.
    • Continue maintenance medications and/or vitamins.
    • No radiographic contrast media (e.g., CT-scan with contrast) a day before the scan.
    • Bring doctor’s request for bone scan.
    • If available, official results of ultrasound, X-ray, CT/MRI, biopsy, PSA (for prostate cancer) and other blood tests. No need to bring the film.
    • If available, images and official report of previous bone scan done outside Chong Hua Hospital.
    • No other diagnostic procedure should be scheduled on the day of examination.
    • Only adults, except pregnant women, are allowed to accompany the patient.
    • The whole procedure may last for 4 hours due to delayed imaging of 2-3 hours from the time of radiopharmaceutical injection.
    • While waiting, patients are encouraged to drink 1 liter of water (except for those with limitation of water intake) and urinate frequently to help reduce radiation exposure to the bladder.
    • Scan time of 45 minutes to 1 hour (patient in supine position with minimal movements)

6. What is the process flow for Radioactive Iodine (RAI) Therapy and Diagnostic Procedures in Nuclear Medicine?

 

        Process Flow for RAI Therapy

o    Consultation with the NucMed Physician.

o    You will be given the preparations along with the quotation.

o    You will be scheduled for your therapy and must pay for the capsule one week prior to the scheduled date.

o     (Inpatient) On the day of your therapy, you will be asked to obtain the Admitting Orders from the Nuclear Medicine Department and present them to the Admitting Section.

o    Fasting for at least 2 hours is required prior to your therapy. The iodine capsule will be given to you in the patient isolation room (6B – Chong Hua Hospital Mandaue; 5A – Chong Hua Hospital Cebu)

o    You will be admitted for at least 3-5 days.

o    Post therapy Scan will be performed prior to discharge.

 

        Process Flow for Diagnostic Procedures in Nucmed

o    To schedule your procedure and further inquiries, you may visit or call the Nuclear Medicine office.

A complete prescription for a dangerous drug contains the following:

  • Date of the prescription – Prescription is valid for 2 months from the prescription date.
  • Patient’s Information – Name, Date of birth and Address
  • Inscription – Only one dangerous drug per prescription, Generic name, Brand name (if any) enclosed in a parenthesis after the generic name and Dosage strength and formulation
  • Subscription – Quantity prescribed (both in words and numerical equivalent), Quantity should not exceed thirty (30) days supply per prescription and other instructions to the pharmacist
  • Transcription or Signa – Dose, Route, Frequency, Duration (if applicable), Other instructions for the patient, the instruction “Take as directed, Take as required, or A.D.” is not allowed.
  • Prescriber’s Information – Name and signature, Complete business address, PRC license, PTR no. (if applicable) and Valid S2 license

The following should be presented to the pharmacy.

  • Special Prescription Form for Dangerous Drug (SPFDD) more commonly known as “yellow prescription” for controlled drugs, or an Ordinary Prescription Form in triplicate copies for regulated drugs
    • 1st of triplicate prescription – Pharmacy copy
    • 2nd of triplicate prescription – Patient’s copy
  • Valid government ID of the patient and/or significant other
  • Authorization letter (if purchased by the significant other)
    • An authorization letter is not required if the patient is not yet of legal age and has a special condition affecting his/her mentality and/or ability to write

6. What should I do with unused medicines?

Unused medicines can be returned within 7 days, except for Cold Chain Drugs.

Unused medicines, if unopened and kept properly stored, may be returned upon presentation of the following:

  • Official receipt
  • Credit Card Transaction slip (for credit card transactions)
  • Valid identification card of the person claiming the refund (for transactions worth 500 pesos and above)

PHILHEALTH Local Health Insurance Offices

1. What outpatient services are covered by PhilHealth (PHIC)?

Out-patients can avail of PhilHealth (PHIC) coverage for the following services:

  • Day Surgeries – including non-emergency surgical procedures ranging from minor to major operations, where patients are safely sent home within the same day for post-operative care (g. Endoscopy, Colonoscopy, Debridement, Ultrasound Guided Biopsy, Eye procedures, Excisions, Ingrown removal, Implant removal, IJ catheter insertion, etc.)
  • Radiotherapy and Chemotherapy
  • Hemodialysis
  • Outpatient Blood Transfusion
  • RT PCR Testing

Note: Immunohistochemistry tests (e.g Cd31, Cd3, Ck7) and all laboratory or diagnostic tests alone

are NOT covered for outpatients.

In addition, admitted patients who meet the Philhealth criteria may also be eligible for PhilHealth (PHIC) deductions.

2. What are the requirements to avail the services of Philhealth?

  • For PHIC members
  • Philhealth Benefits Eligibility Form (PBEF) signed by member

On the day of the procedure, the PBEF with ‘YES’ eligibility will be generated as required by PhilHealth. The patient must present their PhilHealth ID, Member Data Record (MDR), or PhilHealth number—whichever is available—along with a valid government-issued ID for identification purposes. 

  • The Claim Signature Form (CSF) must be signed by the member. If the member is employed but the PBEF eligibility result is ‘NO,’ the CSF must be signed by the employer.
  • Philhealth Receipts if needed (for individually paying members)
  • Marriage Contract, Birth Certificate, joint affidavit, IDs if needed.
  • For NON-Members

The patient must be registered with PhilHealth within the confinement period. Bring a furnished PMRF form and a birth certificate to any PhilHealth branch that accepts membership applications (i.e. SM, Robinsons, Golden Peak, etc.).

3. Process

  • Signing of PHIC forms in behalf of the patient

Supporting documents establishing the relationship to the member/patient must be provided. The representative must also be of legal age.

Sample:

  • Birth Certificate of patient and Marriage Certificate – if patient is minor and if spouse who signed the CSF is not the member
  • Birth Certificate of Member- if parents would sign the CSF in behalf of member
  • Birth Certificate of Member and Sibling- if sibling would sign the CSF in behalf of the member
  • Changing of Civil status

Fill out the PMRF, attach the marriage contract with the registry number, and submit it to the PhilHealth office for updating within the confinement period.

  • Adding additional dependent
  • For Spouse: Fill out the PMRF, attach the marriage contract with the registry number, and submit it to the PhilHealth main office within the confinement period.
  • For children: Fill out the PMRF, attach the birth certificate with the registry number, and submit it to the PhilHealth main office within the confinement period.
  • For a newborn child: Fill out the PMRF, attach a certified true copy of the Live Birth signed by the attending physician, medical records clerk, and parents, then submit it to the PhilHealth office within the confinement period.
  • Refund
  • The hospital has 60 days from the date of discharge to process and submit claims to PhilHealth. Similarly, PhilHealth has 60 days from the date of receipt to review and pay the claim. It is advisable to follow up on the refund after four months from the date of discharge.
  • PhilHealth also informs its members through their registered mobile number when the claim payment has been made.

3. Do I need to bring a prescription for my Radiation Oncology treatment?

Yes, a written prescription from your Attending Physician/Referring Physician/Requesting Physician for us to perform the correct site/correct procedure and safely/appropriately prepare you for the specific treatment procedure.

4. How can I schedule a consultation or treatment with the Radiation Oncology Unit?

  • We accept walk-in patients for consultation from Monday-Friday, 8:00 AM-5:00 PM.
  • For on-going patients:
    • You must follow your assigned schedule of treatment
  • For External Beam Radiation Therapy (EBRT) patients’ first day of treatment:
  • Please wait for the Radiation Oncology Unit staff to call/text you of your treatment schedule.

5. How long do Radiation Oncology procedures typically take?

Procedure duration varies by case. Common treatment lengths include:

  • Common number of days per case (EBRT):
    • Breast Carcinoma: 30-35 days
    • Head & Neck Carcinoma: 33-35 days
    • Gynecological Carcinoma: 28 days
    • Rectal Carcinoma: 28 days
    • Palliative cases: 10-15 days
  • Brachytherapy
    • Gynecological cases: 4 fractions (twice a week at most and 2 days apart)
    • Prostate Carcinoma: 2 fractions per day for 2 days (at least 6-hr gap per fraction)

Note: For Prostate Carcinoma, the patient needs to be admitted.

6. What are the preparation instructions for Radiation Oncology Procedures?

  • All Female patients who suspect pregnancy and or possibly pregnant should notify the Radiation Oncology Staff. Please inform your Physician or technologist if you are pregnant.
  • CT PLANNING/ CT Simulation:
  • Plain CT Procedures:
  • You may eat or drink anything before the procedure. No food restrictions.
  • IV Contrast CT Procedures:
  • Creatinine Result (2 weeks’ validity)
  • 4-6 Hours Fasting is required
  • The Radiation Oncology Nurse may provide other preparation instructions if you have the following medical history:
    1. Food Allergies
    2. Drug/Medication Allergies
  • Bronchial Asthma
  1. Renal/Kidney Problems
  2. Thyroid Problems (provide FT4, TSH laboratory results)
  3. Renal Impairment
  • Renal Surgery
  • Diabetes Mellitus
  1. Hypertension
  • Some procedures may require you to have full bladder (Patient is advised to bring 500 mL of water)
  • External Beam Radiotherapy:

If you are required to have a full bladder during the treatment, a technologist will give instructions on when to drink 500 mL and will let you have to wait for about 20-30 minutes prior treatment.

  • Brachytherapy with Anesthesia
  • Fasting for 6 hours (no food or fluid except maintenance medications)
  • Take omeprazole 2 hours prior to the procedure with sips of water only
  • Be at the radiation oncology unit 1 hour prior to your procedure

Result release times depend on the type of procedure:

  • X-Ray:
    • The result will be read and released within 24 hours after the procedure is performed.
    • For weekend procedures: Results are available by Monday at 5PM
  • Ultrasound:
    • Results are available 15 – 30 minutes after the procedure.
  • Breast Ultrasound:
    • Results are available 30 minutes to 1 hour after the procedure.
  • Scrotum / testes:
    • Results are available 15 – 30 minutes after the procedure.
  • MRI:
    • The official result will be released 24 hours after the procedure.

  • MRI and CT Scan Printouts:

Image printouts will be available after your procedure

  • Soft copy (CD or DVD) of your MRI, CT scan, or Ultrasound

You may request a digital copy from the unit’s office. Burning the images takes 5-10 minutes and costs Php255.00

6. How long does an MRI procedure usually take?

  • For an MRI Brain Plain, the allocated time for the procedure is 1 hour.

Note: The time may vary due to factors such as patient movement, anxiety, and the specific type of MRI being performed.

7. What preparations do I need to make before my procedure?

Preparation may vary depending on the type of procedure. Below are the different types of procedures and their

corresponding preparation guidelines.

  • GENERAL RADIOLOGY (DIAGNOSTIC X-RAY)
    • Routine Diagnostic X-ray Procedures– No preparation needed
    • Barium and Iodinated Contrast – this may require specific patient preparation based on the patient’s medical history and other related health conditions.
    • All Female patients who suspect pregnancy and or pregnant are suggested to notify the Radiology staff. Please inform your Physician or technologist if you are pregnant.
  • CT SCAN/COMPUTED TOMOGRAPHY
    • Routine Plain CT Procedures (non-abdominal procedures) – You may eat or drink anything before the procedure. No food restrictions.
    • Plain CT Procedures (abdominal procedures) – Fasting at least 6 hours (no solid food) (water is okay and medication)
    • IV Contrast CT Procedures:

  • All other procedures: Results are available immediately after the test

5. How should I prepare for my Pulmonary Function Test (PFT) or Spirometry?

To ensure accurate results, please follow these preparation guidelines:

  • Eat light meals before the test.
  • Avoid smoking for at least 12 hours before the test.
  • For first timer patients, please put on hold any Inhalers or Nebulization Medication for:
    • 12 hours for Inhaler Medications
    • 4-6 hours Nebulization of Salbutamol/Ventolin Inhalers
  • Wear comfortable clothing and remove tight belts.
  • Remove any nail polish or nail extensions.
  • Bring drinking water and a doctor’s
  • Make sure you are in good health prior to the procedure, able to walk and sit for extended periods, and not experiencing pain.

6. How can I access the results of my Pulmonary Diagnostic Test?

1. What services are offered by the TB DOTS Unit?

The TB DOTS Unit provides the following services:

    1. Enrollment for TB treatment (free anti TB medications c/o DOH)
    2. Continuation of treatment (for patients enrolled elsewhere who want to transfer to CHH DOTS Clinic)
    3. TB preventive therapy
    4. Diagnostics – sputum AFB smear, sputum GeneXpert processing, PPD (with corresponding fees)

2. What are the requirements for enrolling in the TB DOTS Program?

To enroll in the TB DOTS Program, please prepare and present the following:

  • A National Tuberculosis Program (NTP) Referral form or a referral written on a prescription pad from your attending, referring, or requesting physician
  • Your PHILHEALTH number

If you are not yet a PHILHEALTH member, our nurse can assist you by referring you to the PHILHEALTH personnel inside Chong Hua Hospital.

  • Official test results, such as chest X-ray, sputum GeneXpert, and other laboratory results.

We accept both inpatient referrals (from admitted patients) and outpatient referrals (from the doctors at Medical Mall and non-Chong Hua Hospital accredited physicians).

 

3. What is the schedule for the TB DOTS Unit, and how do I set appointments?

  • We accept walk-in referrals from 10:00 AM to 6:00 PM, with a cut-off time of 4:00 PM.
  • For succeeding visits, you will be scheduled according to your treatment plan.
  • You may also schedule a consultation whenever needed if you experience side effects from the treatment (medication).
 
5. Unit Visit Procedure

TB DOTS Clinic Process Guide
  • Reason for Visit: When you arrive, the nurse will ask about the reason for your visit.
  • For Enrollment: If you are enrolling in the TB DOTS program, please provide your NTP Referral Form or a referral written on a prescription pad by your attending physician, along with your diagnostic results (chest X-ray, lab tests, GeneXpert, etc.).
  • Vital Signs & Registration: The nurse or nursing assistant will check your vital signs and weight and register you in the HIS system. Please present one valid government-issued ID.
  • Orientation & Consent: The nurse will explain the TB DOTS procedure, services, and fees and ask you to sign a consent form for enrollment.
  • Waiting Area: After the orientation, you will wait while the nurse informs the resident in charge.
  • Consultation & Treatment Plan: The resident doctor will conduct a consultation. Afterward, the nurse will discuss your care and treatment plan.
  • Billing & Payment: The nurse will provide a charge slip and guide you to the Billing Office for payment.

(Best option for payment is at 5th Floor Bldg. C OPD open until 5 PM. After 5 PM, proceed to the Main Billing Department at Ground Floor.)

  • Medication & Follow-Up: Return to the center to receive a two-week supply of TB medication and schedule your next follow-up visit in two weeks. You will also receive a TB DOTS brochure for reference.

  • Follow-Up Visits
  • The nurse will retrieve your chart, check your vital signs and weight, and have you wait for your consultation.
  • The resident doctor will assess your treatment progress and any side effects of the medication.
  • You will receive a charge slip for payment at the Billing Office.
  • Return to the center to receive another two-week supply of TB medication and schedule your next visit. This continues until your treatment is completed.
  • Initial Visit (No Referral / Walk-In Patients)
  • The nurse will check your prescription for diagnostic tests (e.g., Gene Xpert) and guide you on proper sputum collection.
  • You will be directed to the Laboratory Department for test payment and specimen collection.
  • Once results are available, return to your attending physician for consultation.
  • Mantoux (PPD) Test Process
  • Present your doctor’s order for the test. The nurse will validate it and direct you to the Billing Department for payment.
  • After payment, return to the center for the procedure:
  • Pediatric patients: The pediatric resident will perform the test.
  • Adult patients: The nurse will perform the test.
  • You must return 48 to 72 hours later for the test reading.
  • You need to buy the PPD reagent from CHH Pharmacy or an outside pharmacy.
  • Visit the unit from Monday to Wednesday to avoid test readings falling on a weekend.
  • Regular TB Medicine Pick-Up
  • Patients are required to visit the center every two weeks for TB medicine refills.
  • During each visit, the resident will assess for side effects or compliance issues with TB medication.
  • If any adverse reactions occur, you will be advised to consult your attending physician.

NOTE: The nurse will call you a day before your next visit as a reminder.

1. How long does it take to process a refund?

  • Check Refund and Card Chargeback processing lead time is normally 5 – 10 Working days from the receipt of the request for refund. Processing lead time includes refund approval, check issuance and chargeback slip preparation, and routing these documents for signature. You may call (032) 255-8000 local 75125 for details of the chargeback.
  • Refund through card chargeback will depend on your respective banks and bank cut-off. This normally takes 2 to 8 weeks for transactions to reflect in your bank statement or credit card statement.

Note: The 5-10 WORKING days lead time is for our internal process (Chong Hua Hospital). The additional 2-8 weeks lead time is for the bank to process the chargeback and the actual reversal to the cardholder’s account.

2. How do I know the status of my refund?

  • You will receive a text message from either Chong Hua Hospital – Fuente or Chong Hua Hospital – Mandaue once check is available for pick-up.
  • You may call (032) 255-8000 local 75125 to check the status of your refund.

3. What are the documents or requirements that I need to bring upon check payment?

  • If you are the check payee, please bring a photocopy of your valid ID.
  • If you are an authorized representative, please bring an authorization letter, photocopy of the payee’s valid ID, and the authorized representative’s valid ID.

4. How to process check or card chargeback refund?

  • To process your check or card chargeback refund, please proceed to
    • Fuente: Disbursement Office on 3rd Floor Building D.
    • Mandaue:
      • Billing office @ UG Floor CHHM Medical Arts Building – Refund NOT related to government assistance
      • Credit and Collection Office @ 5th Floor Chong Hua Hospital Mandaue – Medical Arts Building – Refund related to government assistance.
  • Once you receive the check:
    • Check payee may encash the check at any METROBANK.
      • The nearest MBTC Bank is located at Chong Hua Medical Mall located at the back of Chong Hua Hospital in Fuente.
      • The nearest MBTC Bank is located near Chong Hua Mandaue is located at Parkmall.
    • Check payee or authorized representative can deposit the check to the payee’s savings account in any bank.

5. Other Instructions

  • How do I process my refund?
    • You can process cash refund if the refundable amount is P20,000.00 and below for payments received in cash, bank transfer or check payments. The refundable amount of more than P20,000.00 is already in the form of a check.
  • Can I process a Cash/Check refund if the payment has been made through Debit or Credit Card?
    • Yes, but a 3% service charge will be deducted from the refundable amount. The service charge pertains to the rate deducted by the bank for debit/credit card transactions. 
    • For a refundable amount of P20,000.00 and below, cash may be claimed from the billing cashier.
    • For a refundable amount of more than P20,000.00, a check will be issued.
  • Can I use my refundable amount to settle my current bills/payable (ER, In-patient, Out-patient accounts) to Chong Hua?
    • Yes, we can off-set your refundable amount to your current bill/payable.
    • If refundable is attributable to government assistance
      Please proceed to the Disbursement Office at 3rd Floor Building D for CHH – Fuente or to the Credit & Collection Office at 5th Floor CHHM Medical Arts Building for CHH – Mandaue.
      You will be given the Patient’s SOA with approval for Refund Offset. Once you receive this, you may proceed to Billing/Cashier to facilitate the Refund offset.
    • If refundable is attributable to your own payment, you may proceed directly to the billing section to request off-set to your current bill/payable.
  • Does my refund have an expiration date?
    • No. As long as the account can still be accessed and validated in the system. We can process the refund any time.
  • Can I get the excess payment from the government assistance paid to my account even if I do not have any payment to the hospital?
    • Yes, only if you provide supporting documents (medicine receipts, payment to doctors etc.) within the period indicated in your statement of account covered by the government assistance or if you get confirmation from the government agency issuing assistance that excess payment to the hospital will be given to the patient and will not be returned to the government agency.
    • Another option, that you can benefit from the excess payment is to offset the refundable amount to your future laboratory, out-patient, in-patient, ER or Pharma transactions.
    • Please note, government agencies will normally issue a Letter of Guarantee only to the extent of the patient’s outstanding balance to the hospital. It will not issue more than the patient’s payable to the hospital.
